Athletes are required to have the forms located below this section on file with the athletic office prior to taking part an any sport specific practice.  All athletes must also meet the OHSAA eligibility qualifications to participate in any extracurricular activities. 

 

Note:  Physicals expire one year after first received.

 

 

Information regarding OHSAA Eligibility 

 

 

Mason City School District will be providing transportation to or from this activity. However, due to circumstances, my child will not be able to utilize this option.

 

Mason City School District will not be providing transportation to or from this activity. Thus, if i choose to allow my child to participate in this activity, I acknowledge that it is my responsibility to ensure that my child is provided safe transportation to and from this activity. 

 

Information regarding Volunteer Drivers 

 

 

This form is to verify that William Mason High School has permission to release a transcript of grades and/or college board (ACT or SAT) test scores regarding the student athlete to any coach/recruiter that requests this information, until the student graduates.

 

 

Athletes are required to pay a fee for each sport they play, look here to see when and how much is to be paid for your students sport.
